a proposito di Damage Control Surgery (corso di aggiornamento), guerra tra medici (anestesisti/chirurghi)

RISPOSTA NON SODDISFACENTE CHE ULTERIORMENTE COMPLICA LE COSE, SE QUESTO DEVE ESSERE IL MODO CON CUI COLLABORARE ESSO E' INSODDISFACENTE.
AVREI PREFERITO UN'ALTRA RISPOSTA PIU' COLLABORATIVA E CHE PIU' FOSSE ENTRATA NEL MERITO DEL CLAIM QUANTO ALLE ASCE DI GUERRA ANCHE QUI SI EVIDENZIA UN MISUNDERSTANDING , LE ASCE ERANO DISSEPPELLITE DA CHIRURGHI ED ANESTESISTI CONTRO LA MALATTIA, UNA PRIMA VOLTA PERMETTENDO AL CHIRURGO STRAORDINARI INTERVENTI ED UNA SECONDA CON LA MEDICINA PERIOPERATORIA.
MA VA BENE UGUALE, IL MESSAGGIO DEL DIRETTORE .... E' CHE OGNUNO DEVE CORRERE LA SUA CORSA. SIAMO INDIETRO, MOLTO INDIETRO MA VA BENE UGUALE QUANDO NON SI VUOLE PERDERE LA FACCIA A TUTTI I COSTI. BASTAVA DIRE ABBIAMO SBAGLIATO E RIMEDIEREMO INSIEME, INVECE SI CERCA LO SCONTRO A TUTTI I COSTI ,LA DELEGITTIMAZIONE, L'INTERPRETAZIONE CAPZIOSA, E ALLORA LO SCONTRO SIA SE E' TANTO DESIDERATO.

Reducing expenditure on low clinical value treatments, risparmio in chirurgia

Il risparmio in chirurgia e' possibile!

Il sistema sanitario inglese ha commissionato uno studio per verificare in quale settore chirurgico e' possibile risparmiare.

"The NHS in England is expected to make efficiency savings of £20bn over the next few years. In February 2009 McKinsey were commissioned to outline how savings might be achieved. One area where McKinsey suggested savings might be made were surgical procedures with "limited clinical benefit."

McKinsey described hernia repairs as "potentially cosmetic interventions". They proposed that reducing the numbers of inguinal, umbilical and femoral hernias operated by 25-50% would save £24.8-£49.5m. Similarly, reducing incisional and ventral hernia repairs by 10-75% would save £3.4-25.5m.

An Audit Commission report published this month looks at how savings can be made on "low clinical value treatments". The report focuses on Croydon PCT's list of "low priority treatments" that has been adopted by other commissioners too. Under the same heading "potentially cosmetic procedures" this retains reference to reducing all hernia repair rates."

Gli interventi chirurgici per ernia (inguinale, crurale, laparoceli) sono considerati "potentially cosmetic procedures" e quindi possono contribuire ad un risparmio notevole se si evitassero (salvo quelli con complicazione).

Gli addetti ai lavori sanno che il nostro SSN segue nel tempo le iniziative del NHS inglese per cui tra 5 anni, anche in Italia, saranno ridotti gli interventi chirurgici per ernia (risparmio sull'uso di protesi).

Attualmente si aspetta 1 anno (12 mesi) per l'intervento chirurgico di riparazione dell'ernia! Quelli che hanno una assicurazione privata (libera professione, paganti) aspettano 1 settimana (7 giorni)!
